A 62-year-old man was referred for elective coronary surgery. He had an acute coronary syndrome that resulted in inferior myocardial infarction complicated by supraventricular tachycardia 1 year earlier. The left anterior descending and right coronary arteries were stented successfully. One month before admission, his symptoms recurred. Catheterization showed significant in-stent stenosis of the left anterior descending artery and severe mitral regurgitation.
