A 55-year-old man (nondiabetic, smoker) presented with dyspnea on exertion that had gradually progressed over 2 years. Examination revealed normal blood pressure. Auscultation was unremarkable except for a loud first heart sound. Transthoracic echocardiography showed an aneurysm from the noncoronary sinus of Valsalva, protruding into the left atrium (Figure 1).This mass was pressing on the anterior mitral leaflet, creating a transmitral gradient of 20 mm Hg, with mild mitral regurgitation. No thrombus was visualized in this unruptured cystic mass with clear borders. The patient underwent surgical repair of the lesion.
