Acute coronary syndrome due to coronary artery-pulmonary artery fistula

A 44-year-old man presented with acute coronary syndrome. An angiogram (Figure 1) showed the extremely rare finding of an aberrant left anterior descending coronary artery branch fistulizing into the root of the pulmonary artery, causing a steal phenomenon. This was confirmed on computed tomography (Figure 2). Angioplasty was unsuccessful, hence the fistula was ligated surgically on-pump.
