A 30 year-old right-hand dominant gentleman was admitted to our department with inability to extend his right thumb, which had developed while playing a video game the previous day. He played video games regularly for 8 to 10 hours each week and had done so for more than 20 years. He had no history of wrist injury or injection to the thumb or wrist. Physical examination of the right thumb showed diminished power of extension at the metacarpophalangeal joint and no extension power at the interphalangeal joint.
