What in the world, you might ask, does Swiss cheese have to do with techniques and technologies for mitigating error in materials handling? Granted, iVT is known for the odd piece of lateral thinking - but Swiss cheese? The variety with numerous holes? Yes! The 'Swiss cheese model' of accident causation was named and popularised by world leader in error and accidents, Professor James Reason of the UK's Manchester University. The model has been used successfully to reduce accidents and system failures in settings as diverse as medicine and aviation.
