摘要

Diabetes is a major risk factor for increased cardiovascular morbidity &mortality rates [1]. The myriad metabolic derangement that accompany diabetes appear to have adverse effect on myocardial tissue & ultimately its function, shifts in energy production that occur with diabetes lead to lipotoxicity, apoptosis, and replacement of myocardial tissue with fibrosis tissue. These pathologic response leads to loss of myocardial function, initially with diastolic filling, but ultimately with systolic function as well. Along with hyperglycaemia, advanced glycosylated end product, altered RAAS activation, mitochondrial dysfunction, epigenetic changes, microangiopathic effect & cardiac autonomic neuropathy play key role in development of myocardial dysfunction. B type natriuretic peptide is a marker of heart failure & NT pro BNP is more specific.
