A 6-year-old boy with biliary cirrhosis developed cyanosis acutely. The child had a history of biliary atresia, and a Kasai procedure was performed during infancy. The chest radiograph demonstrated a new reticulonodular interstitial pulmonary pattern. Technetium-99m macro-aggregated albumin lung imaging was performed to determine if intrapuimonary vascular malformations were a complicating factor in the child with chronic liver disease. The lung image documented intrapulmonary fistulae. This is a rare complication in surviving children with biliary atresia. This case illustrates the value of Tc-99m particle imaging in this condition.
