A 32-year-old man had radionuclide scrotal venography to identify a possible varicocele. Our standard method did not reveal this. Because an ultrasound examination was equivocal and clinical signs continued of a varicocele, we repeated the study with the Valsalva maneuver, which revealed a left-sided varicocele. Our observation showed that even a varicocele of grade 2 or 3 could be missed by radionuclide scrotal venography if the Valsalva maneuver is not applied.
