A patient with large cell lymphoma, diagnosed by excisional biopsy of an axillary lymph node, underwent Tc-99m sulfur colloid liver-spleen scintigraphy because of increasing serum bilirubin and higher liver enzyme levels. In addition to massive hepatosplenomegaly, the images showed diffusely increased uptake in the lungs and bone marrow of the skull, spine, clavicles, and long bones. These findings may reflect the expansion of hematopoietic activity secondary to lymphoma cell infiltration of the bone marrow, which was shown by an iliac crest biopsy. Diffuse uptake of the lungs and massive hepatosplenomegaly may indicate extramedullary hematopoietic activity.
