In this issue of Breast Care a number of articles shed light on the side effects of therapies in the treatment of breast cancer. This is an important issue in the decision-making processes aiming at a fair tradeoff between benefit and risk for the individual patient. In its essence this merging of knowledge of evidence based medicine from the literature, the oncologists' personal experience with specific comorbidities and the problems of the individual patient is at the very heart of medicine. The quality of treatment decisions is the core of the art of oncology and must be steadily adapted to the development of the field.
