摘要

A 71-year-old gentleman was referred with a 6-month history of progressive leg weakness, altered sensation, beginning in his feet and discomfort in his calves. He also reported a band-like pain across his chest and back which had been worsening over the last year, and weight loss. His background included Raynaud's phenomenon, recurrent uveitis and psoriasis. Examination revealed diffuse areflexia and weakness of ankle dorsiflexion, foot inversion and eversion, and toe extension bilaterally (Medical Research Council grade 4/5). The remaining motor examination was normal and sensory exam elicited impaired pinprick, light touch and vibration sensation below the knees. Joint position sense was normal. His gait was slightly hesitant with minimal foot drop.
