People often ask me why level crossing accidents happen. My usual answer is,'If you've got a spare hour and a half I will tell you.'At least 95% of the time the fault is with members of the public. I can only think of two occasions when it was not. One was up north somewhere where a train driver overran a red light onto a level crossing. The other was at Clinton during a heavy snowfall when a railcar failed to activate the crossing alarms because snow prevented the track circuit from tripping.
