When we are very busy it can be hard to fit in time for team learning. Colleagues in East London have written up experiences of 'Flash teaching'; 5-minute weekly discussions. One useful framework that produced change in team behaviour was the 'Handy Approach' -a framework using the hand as a memory tool to do a very brief assessment of personal goals for care of all patients referred to a Virtual ward'. Many frailty assessment tools are cumbersome, and, in my experience, still result in the patient returning to discuss what really matters to them.
