A Birmingham construction firm has been fined ?100,000 after a worker fell five storeys to his death while chasing a piece of tarpaulin. Birmingham Crown Court heard on 5 May that H20 Plumbing Services failed to carry out risk assessments or provide a method statement to workers who were repairing the lift housings on the roof of an apartment building.
