New research over the past two decades on the prostatitis syndromes has led to a further understanding of the pathogenic basis of non-bacterial prostatitis and prostatodynia, a relook at our classification system and a rationalization of our diagnostic and treatment regimes. The optimal approach to the management of this syndrome is to rule out infectious causes for the symptoms and then differentiate the inflammatory from noninflammatory conditions. Based on the latest research, some hypothesis, common sense, and personal opinion, this paper presents a simple approach to the diagnosis and treatment of non-bacterial prostatitis and prostatodynia.
