A 54-year old man, mechanically ventilated after cardiopulmonary resuscitation secondary to constrictive effusive metastatic pericarditis of primary lung cancer, developed acute kidney failure. Creatinine increased from 2.4 to 8.8?mg/dl in 10 days, with concomitant hyperkalemia (serum potassium=6?mEq/l). Treatment of the hyperkalemia included continuous insulin infusion, beta agonists, and 30?g of sodium polystyrene sulfonate (SPS)-sorbitol free, three to four times a day, for a duration of 10 days, administered via a nasogastric tube
