Patient care leaders are moving forward to advance their population health management strategies. How does the landscape look now, compared to a few years ago? First, I would note that the COVID-19 pandemic has provided pop-health leaders with both enormous challenges, but also great opportunities. At the height of the pandemic, very few patients were being seen in-person in ambulatory settings, so apparent performance in quality programs for chronic conditions fell off of a cliff! If we don't have any BP readings or lab results because patients aren't leaving their homes, it appears we are not managing the health of the population.
