It's usually observed a lack of awareness related failure equipment and the need of in depth investigation to define causes of damage and preventive actions. Commonly, the steelmaker is more engaged to produce steel responding to customers requirement than thinking how to tackle occurrences of failures of mechanical parts. It's known that equipment failure highly impacts production costs in repair or replacement.Regarding to the complexity and the diversity of failures and their effects in increasing cost production, investments in concept for saving money is highly motivated. The concept should be supported by firstly a general procedure for determining causes of the damage observed. This procedure lead to define failure causes and to point clearly the responsibility of each actor. It intends to propose measures to avoid recurrent failures costs. Damages and failures may appear on product during manufacturing or transformation on equipment and consumables materials. A systematic process for managing failure investigations is required to undertake prevention actions. This analysis process must be carried out by qualified services.
