Author Reply: Partial Breast Intraoperative Radiation

We thank Dr Ziogas et al.for their comments on our work.Currently, we do not think that young women with extensive DCIS, multifocal, or multicentric breast tumors should be offered lumpectomy and intraoperative radiation therapy (IORT). Their chance of having positive margins, residual disease, and local recurrence is considered too high for this modality. We agree that further trial data should be obtained before concluding that partial breast irradiation, given intraoperatively or by external beam, is equivalent to standard whole breast radiation. We "Support the targeted intraoperative radiation therapy (TARGIT) randomized controlled trial that examines this question in relation to IORT and the Canadian randomized trial of accelerated partial breast irradiation (RAPID) trial that is examining partial breast irradiation. Adoption of these techniques into practice is ideally based on sound trial evidence.
