Having led the commissioning of a new service to provide individual dietary interventions to patients with type 2 diabetes, I agree with most of Clifton's conclusions. In ourtwo year pilot, a dietitian provides individual dietary advice to patients at six rural surgeries. Glycated haemoglobin (HbA_(1c)) readings dropped by 0.9 after the first six months and 0.6 after a year (mean starting HbA_(1c) of 8.2 and 7.8, respectively) in patients seen by the dietitian. Most reductions were in patients who were already on diabetic drugs. These results agree with those reported by Coppell and colleagues.
