Teaching NeuroImages: Acquired hepatocerebral degeneration: An underrecognized complication of advanced liver disease

A 42-year-old man with alcoholic cirrhosis presented to a neurology clinic for action tremor, attention/concentration deficits, and parkinsonism after hospitalization for alcohol withdrawal and acute alcoholic hepatitis. A brain MRI showed bilateral T1 hyperintense lesions in the globus pallidus with concomitant T2 hypointensity (figure).
