Epilepsy Centers in the United States:The Times They Are A-Changing

The modern era of epilepsy surgery began at Queen’s Square in the 1880s. Resections were directed by semiology alone and outcomes were improved through a meticulous surgical approach. The first EEG recordings of interictal epileptiform discharges were recorded in the 1930s. During the 1940s and 1950s, interictal EEG findings were used to plan temporal lobectomies. In the 1970s, the advent of CT imaging and routine preoperative video EEG monitoring revolutionized the approach to the evaluation for epilepsy surgery. Subsequent developments of MRI, PET, laser resections, stimulation therapies, and other advances have continued to optimize preoperative and surgical approaches in the management of drugresistant epilepsy (DRE).
