NHS trust fined ￡200,000 for maintenance failures

Royal Berkshire NHS Foundation Trust has been fined ￡200,000 for failing to manage and maintain hospital trolleys properly. The HSE investigated the trust after an elderly patient fell from a trolley and later died. Major James Fyfe, who was 90, broke his neck in the fall from an Anetic Aid QA3 trolley on 27 March 2011 at the Royal Berkshire Hospital in Reading. Sharron Cripps, the HSE inspector who investigated the case, said Major Fyfe had arrived at the hospital in an ambulance to have a suspected fractured hip x-rayed. He fell from the trolley, which had moveable sides that were not adequately secured, while in the x-ray department. The hospital treated him for his injuries but he died 26 days later.
