In this feature, we look at longstanding machinery guarding failures that caused the fatal entrapment of a worker and resulted in his employer admitting a charge of corporate manslaughter. The case, as with most of the previous 24 sentences for corporate manslaughter, involves a small organisation, with the hands-on involvement of a director (who received a suspended prison sentence for an HSW Act offence). It also arose from cost cutting at the expense of safety and resulted in a significant fine - for a micro-company - of ￡255,000, even though it is in liquidation.
