In this feature, we look at the failures that led to a judge fining Costain ￡1.2 million for failures that caused a reinforcement cage to collapse onto a mobile elevated working platform (MEWP), severely injuring two workers on the MEWP. The fine represents 40% of the company's most recent annual pre-tax profit. A second company, Brenbuild, was fined ￡80,000 for its failures, which were also a significant cause of the incident.
