Health visitor standards

I am a health visitor and I am part of a working party looking at creating a standard for record keeping. We recently completed a records audit, which revealed a significant difference in recording each contact within our heath visitor teams. We are looking to establish a standard for what each entry should include, based on observation, evidence and the implications of the impact of these conditions on the child/children, while at the same time not wanting to prolong the procedure of writing up each contact.
