Sporadic Creutzfeldt-Jakob disease mimicking nonconvulsive status epilepticus.

To the Editor: We read the article by Lapergue et al. about sCJD presenting as NCSE with interest. We observed a 53-year-old man and a 76-year-old woman with presumed NCSE who both died from sCJD confirmed on postmortem neuropathology. Both cases presented with an alteration in baseline mental state, had initial EEG findings consistent with initial lateralized sharp waves about 1 Hz, and were considered in NCSE. Despite AED treatment, they both progressed from awake and verbally interactive to stupor and coma within 3 months of initial presentation. EEG patterns in both cases began as lateralized to one hemisphere but became invariably generalized and showed 1- to 1.4-Hz PSWC characteristic for sCJD.
