Suspected anaphylaxis after intravenousadministration of cefuroxime (Zinacef) in two dogs,with descriptions of arterial blood gas abnormalitiesoverotime

Two cases of suspected anaphylaxis occurred in theauthors’ hospital within two weeks of each other.The first, a six-year-and-four-month-old border collie,presented for thoracic wall resection. Once anaesthetised,20mg/kg of intravenous cefuroxime (Zinacef 75mg/ml, GlaxoSmithKline UK) was administered slowly forprophylactic antibiosis. Ten minutes after administrationwas completed,desaturation occurred alongsidetachycardia, hypotension and apnoea. Chest compliancewas extremely poor, with minimal chest movement.Treatment included intermittent positive pressureventilation, intravenous fluids, terbutaline, adrenaline anddexamethasone. The second case, a five-year-and-eightmonth-oldLhasa apso, presented for elective orthopaedicsurgery. Intravenous cefuroxime (Zinacef 75mg/ml) wasagain administered slowly for prophylactic antibiosis;however, it is uncertain whether the patient received thefull dose of 20mg/kg. Mid-administration tachycardia,tachypnoea and hypotension occurred,alongside poorchest compliance. Cefuroxime administration wasstopped, the fluid line disconnected and treatment withchlorphenamine given.
