Interventions to decrease hospital readmission rates: who saves? Who pays?

ABOUT 20% OF HOSPITALIZED MEDICARE PA-tients are readmitted within 30 days. Several randomized controlled trials (RCTs) have shown that it is possible to reduce hospital readmission rates through improved discharge planning and coordination and post-hospitalization care. Decreasing hospital readmissions offers the hope of improving care while simultaneously reducing health care costs.
