Permissive Hypercapnia for ARDS: New Evidence Supports Its Use

Fifty-three patients with severe adult respiratory distress syndrome (ARDS) were managed with a pressure-limiting strategy and permissive hypercapnia (tidal volume, 4-7 cc/kg; peak airway pressures maintained between 30 and 35 cm of H-20). The average PaCO-2 reached was 67 mmHg. Hospital mortality was significantly lower than predicted either by APACHE II or number of organ failures (26% versus 54%), and no overt barotrauma was detected.
