We thank Terrace and Wigmore for their comments. The reported incidence of pancreatic leak after pancreati-coduodenectomy remains high, despite a number of procedural modifications. Multiple series have reported leak rates greater than 10% and in high-risk patients, this rate will rise significantly. The onus is on pancreatic surgeons to develop new techniques to reduce this leak rate, the subsequent morbidity and the cost to the health system.
