Coexistence of thyroid metastasis carcinoma in the background of HT and primary thyroid mucosa-associated lymphoid tissue B cell lymphoma in a thyroid gland.

A 60-year-old woman was admitted to local hospital with a painless mass in the left cervix for 10 months. Her past history included a combined abdominoperineal resection (Mile's operation) 1 year ago for a rectum moderately differentiated Dukes' C adenocarcinoma with a preoperative carcinoem-bryonic antigen (CEA) level of 13.90 ng/ml and a postoperative CEA level of 4.15 ng/ml (normal <5 ng/ml). Moreover, she has a 3-year history of diabetes mellitus. The results of X-ray, ultrasonography, and CT showed normal but a diffused enlargement in the left lobe of thyroid. The gastroscopy and bone scintigraphy were also normal. Thyroid function screening test showed free T3:6.61 pmol/1 (normal 2.50-9.82 pmol/1), free T4:8.80 pmol/l(10.0-25.0 pmol/1) and sensitive thyroid stimulating hormone (sTSH) 17.44 mu IU/ml (0.30-5.0 muIU/ml). Thyroid globulin auto-antibodies (TGAb) was 45% (normal <30%), and thyroid microsomal antibodies (TMAb) was 35%( normal <20%), thyroid peroxidase anti-bodies(TPOAb) was >l,000 KIU/L (normal <35 KIU/L).
