In the most recent report, PHSO has selected nine of the most serious of its cases to illustrate the devastating impact that failures in hospital and social care discharge planning can have.The PHSO talks about the untold anguish such poor discharge arranges can have on families and carers. Tragically, some of the patients in the case reports died because of poor discharge planning, and their families want to know what has been learned from these poor care experiences and what will happen next to make things better for future patients.
