Ranolazine, a piperazine derivative, is used for the treatment of chronic angina pectoris (7). The anti-ischemic and anti-anginal effects of this drug have been extensively studied both in laboratory animals (27, 51) and in patients with coronary artery disease (24, 42, 44). Ranolazine, unlike other anti-anginal drugs, is clinically effective at plasma concentrations of 2-6 |xM that do not appreciably change the rate-pressure product. Thus it has been proposed that the mechanism of action of ranolazine is independent of its effect on myocardial oxygen consumption (MV02). In these clinical studies, other determinants of MV02, such as cardiac contractility and preload, were not measured.
