Occluded Portex Epifuse? epidural connector

I read with interest the letter from Richardson et al., regarding their experience with the Portex Epi-Fuse~(TM) connector [1]. I wish to report a further incident at my institution. The introduction of the EpiFuse connector (Smiths-Medical, Kent, UK) was intended to prevent accidental disconnection of the epidural catheter from its antibacterial filter. The new mechanism involves inserting the epidural catheter into the connector until resistance is felt, then clamping the connector around the epidural catheter.
