During anaesthesia of a 67-year-old male undergoing cervical laminectomy at C3/4, an Alaris PK Mk III pump (CareFusion, Basingstoke, UK) delivering remifentanil alarmed 'PL3' when the infusion was recommenced after a new syringe was fitted. The pump would not recommence target- controlled infusion (TCI), necessitating a change to a manual infusion.
