Although there are several complications attributed to thoracic paravertebral block, there have been no previous reports of vocal cord palsy. A 65-year-old, 50 kg male presented with haemopneumothorax with right-sided second to fifth rib fractures. An intercostal drain was inserted and a continuous thoracic paravertebral block was planned for pain relief. An 18 G epidural catheter was inserted in right third thoracic paravertebral space using a classical technique for thoracic paravertebral block with the patient in the sitting position, with 3 cm of catheter left inside the space. After a test dose, 15 ml 0.5% ropivacaine (0.3 ml/kg) was given with a plan to start a continuous infusion after half an hour. There was sensory loss in the T2-T6 dermatomes with stable haemodynamic variables. After 25 minutes, the patient was having difficulty speaking.
