The National Public Health Week theme, "A Healthier America: One Community at a Time," is a call to action that can only be realized in the context of achieving a healthier world. In this era of global health, we confront diseases in a world where borders and geographic distances are increasingly irrelevant to the agents of disease in our "global village."In the United States and elsewhere, we must continue to raise a greater concern about the inequalities and disparities in the social determinants of health that afflict those in less advantaged communities. Poverty leads to a vicious cycle of disease, lower productivity, even greater poverty, and so on. Individuals mired in poverty serve as reservoirs of disease that can affect us all. As Nobel Laureate Joshua Lederberg once stated,
