Dialysis vintage could confound survival trends in ESRD patients

Ohlsson et al describe 4 patients with anti—glomerular basement membrane (anti-GBM) disease and immunoglobulin G4 (IgG4)-restricted autoantibodies.1 Remarkably, anti-GBM enzyme-linked im-munosorbent assay demonstrated that 2 of these patients had circulating autoantibodies that bound more strongly to antigen coated under native conditions then to guanidine-dissociated antigen. This is atypical of Goodpasture autoantibodies, suggesting different epitope specificity.
