Since the 1950s, there has been a flickering candle of hope that allogeneic organ transplant could be undertaken without the need for chemical immunosuppression. If only we understood better how to evade the alloimmune response or to suppress it, delete it, or regulate it. For the most part, the waves of experimental nomenclature and conceptual paradigms have been dashed against the rocks of reality: what works in a small animal does not work in a large one, or what works in a large animal is too risky for a clinical trial. However, there are 3 standout clinical programs that show that the flickering candle of hope is not extinguished.
