New 2011 updated DHHS antiretroviral treatment guidelines and chronic hepatitis B.

The new updated 2011 DHHS guidelines for anti-retroviral treatment of adults with HIV infection [1] represent an important step for setting up the optimal management of HIV-infected persons at a time when a broad number of potent and relatively well tolerated antiretroviral agents is available and when the deleterious impact of inflammatory processes associated with uncontrolled HIV replication regardless of CD4 cell counts has been fully appreciated. Moreover, given the accelerated course of liver disease in the subset of HIV-infected patients with chronic hepatitis B [2], these guidelines recommend earlier initiation of antiretroviral therapy in most HIV-HBV-coinfected individuals. Three aspects, however, merits some consideration.
