to the editor: Dr. Koning et al.'s (1) questioning of the appropriateness of nonpulsatile cardiopulmonary bypass is to be welcomed even more, because the study is strongly influenced by a dogma, which abandoned manifestation was the windkessel model. The latter presented the large arteries, as capacitor to serve another dogma that the circulation primarily needs continuous oxygen supply. We have this impression, because the authors avoided questioning why evolution had come to the pulsatile solution. Otherwise, it is appreciated that they focused on the microcirculation as the most affected, we say, in supine, while in upright it is absolutely not the same. To clarify the latter, when a dog with nonpulsatile flow is erected, it instantly dies. Why? The dogma is silent, but this silence is horrible for the clinic.
