Nurse Practitioner Home-Based Primary Care Program Improves Patient Outcomes

The Health@Home program was implemented in 2014 by a New England managed care organization with a large Medicaid membership. It provides comprehensive primary care in the homes of patients with complex medical, social, and behavioral conditions for whom routine office-based care has not been effective. Health@Home is a team-based program led by nurse practitioners experienced in working with adults with chronic conditions. Evaluation of the first 2 program cohorts was promising, demonstrating short-term and long-term effects in reducing rates and associated costs of medical and behavioral health inpatient hospital admissions and emergency department visits.
