Beware temporal arteritis that does not respond promptly to steriods.

A 94-year-old man presented with progressive right-sided headache and scalp tenderness following an 'ear infection'. Erythrocyte sedimentation rate was 79 mm/h. A standard non-contrast computed tomography (CT) brain scan was normal. He was treated with oral steroids for presumed temporal arteritis, having been judged too frail to undergo biopsy at that time.
