Periprostatic nerve block (PNB) alone vs PNB combined with an anaesthetic-myorelaxant agent cream for prostate biopsy: a prospective, randomized double-arm study.

We read with great interest this article by Cantiello etal. [1], which significantly adds to the already abundant reports on the timely and debated topic of pain relief during TRUS-guided prostate biopsy (PB). Although PNB is currently considered the standard for pain control during PB [2,3], it does not eliminate the probe-related anorectal discomfort of the procedure, and might even add considerable pain due to the transrectal infiltration of local anaesthetic [4]. Thus, the potential role of combined analgesia, consisting of topically or systemically administered drugs aimed at anorectal discomfort relief (i.e. local anaesthetics, myorelaxant agents or analgesics) and PNB, is increasingly explored in randomized controlled trials (RCTs) [5-10].
