摘要

There is no current consensus in biomechanics regarding the definition of a "custom foot orthotic," and there is no clear understanding of how they work. There is also no standardization in terminology and methodology in researching and practicing biomechanics. Evidence is revealing that excessive impact forces and pronation are not major factors in the development of overuse syndromes and deformities. Subtalar joint neutral is an old concept, but is no longer gold-standard. Neoteric concepts need to be developed, researched and practiced to improve the understanding of anatomical and biomechanical abnormalities and their value in optimizing efficient, injury-free human stance and movement. The Foot Centering Theory of Structure and Function is a qualitative biomechanical paradigm with quantitative elements, which starts by classifying all feet into one of five common foot types in order to expand custom care and provide better outcomes. All feet are sub-grouped based on their inherited structure and common characteristics, strengths and weaknesses, allowing for foot type-specific diagnosis and treatment that then allows aggressive care and further customization on a case-by-case basis.
