Endometriosis has been related to menstruation since Sampson formulated the hypothesis that menstrual regurgita-tion and implantation of endometrial debris are the principal causes of the disease. If we accept this hypothesis, its onset cannot precede menarche when endometrial shedding and menstrual bleeding begin to occur. However, endometriosis has been documented in normal young girls before menarche and, in these cases it has been assumed that its pathogenesis and pathophysiology differ from adolescent and adult endometriosis.
