A significant nationwide concern to deal with clean air objectives, environmental problems and legal requirements has revealed an increase in the use of LPG as an automotive fuel. Successful expansion of newer technologies depends on the development and dispersal of production, storage, handling and refuelling infrastructures collectively with precise safety assessment studies to suitably devise the safety management systems for loss prevention in these plants. Hazards due to human errors are a significant cause leading to accidents, in complex technical environment. This paper attempts to systematically foresee the human (operator) error potentials for designated tasks (dispensing operation) in an LPG refuelling station by applying a second generation human reliability assessment tool, CREAM (Cognitive reliability and error analysis method). The research also contributes to quantify the probability of various human errors by applying basic and extended versions by cognitive approach.
