Transition to adult services of youngpeople with palliative care needs is thecrisis riding the wave of advances in paediatricmedicine. The review by Douget al has highlighted the extent of thecrises we are facing, with a lack of standardisedtransition programmes, a poorevidence base to transition guidelinesand insuffi cient outcome data to evaluatethe different models. How, in palliativecare, have we found ourselves in thisposition, with a long-anticipated patientpopulation that has no clear destinationand little means of getting there?
