Nasal Granulomatosis as an Extraintestinal Manifestation of Crohn Disease

A 51-year-old woman with colonic Crohn disease (CD) presented with an exacerbation of chronic nasal congestion, as well as dysosmia, unilateral headaches, and facial pain of 4 weeks' duration. She had experienced similar symptoms intermittently for the past 10 years without significant impact on activities of daily living. She had made several visits to oto-rhinolaryngology specialists, whose prescribed treatment regimens-including amoxicillin clavuianate, methylprednisolone dose packs, saline nasal sprays, and nasal corticosteroid nebulizers-had resulted in limited relief of the patient's nasal congestion symptoms.
